 For clients under 17, a legal guardian must complete the  intake form/waiver.

• Occasionally, appointments may exceed their allotted time. We appreciate your patience, and rest assured, you will receive the same excellent care once your service begins.

• To maintain efficiency, please arrive a minimum of 5 minutes early. and come to the door at your appointment time.

• Cancellations, no shows or rescheduling made with less than 24 hours notice will require payment of the full service booked. Over 10 min late is considered a no show.

• For the comfort of all clients, please attend your appointment alone, as we do not have seating or space for spouses, children, or friends.
